MedsReducer-Physician is a tool that helps a doctor to personalize a patient’s lithium maintenance dose. The doctor can slowly modify the daily dose by less than ten percent of the patient’s current dose using steps even smaller than the minimum dose capsules or tablets available, without cutting any capsules or tablets. The app displays a short daily dose pattern having a fixed average daily dose over a cycle no longer than 5 days.
As a patient ages or develops comorbidity, such as decreased renal function, the lithium dose will need to be gradually modified. Modifying in the small steps suggested by MedsReducer-Physician means that the blood serum level can be kept at the right level. This is ideal for avoiding toxicity for the elderly.
How to use MedsReducer-Physician
1. Once MedsReducer-Physician App has been downloaded to a mobile device the user will be required to register. Registration requires filling in simple information such as email, name, and a password.

2. Once registered, users just need to log in using email and password to use MedsReducer-Physician and purchase a subscription. Terms of use can be found at: https://medsreducer.com/terms-of-use.

3. Input the patient’s current daily dose. The currently daily dose pattern is displayed, including the time to take the medication each day, and how much to take at each time.

4. The physician then selects the button labelled “New Avg. Daily Dose” and is provided with a list of average daily doses not more than 10% different from the current daily dose.

5. The pattern for the selected new average daily dose is then displayed. The daily dose in the pattern may vary over a short cycle of days but the average daily dose is fixed over a cycle no longer than 5 days. The display lists the daily doses for each individual day in the cycle with a choice of administering the lithium once a day, twice a day, or three times a day.

6. The physician can try several different modifications to review the dosage patterns. The decision as to which dose to use, is entirely up to the physician.

7. When the physician has chosen the new average daily dose, they can use the “Assign” button to assign the dose to a patient.

8. The physician can modify the starting date for the new average daily dose, by touching the date and selecting a new date from the calendar.

9. To assign the dose the physician needs to obtain a Share ID and Response ID from the patient. These ID’s are displayed on the Profile page of the patient’s MedsReducer-Patient App. Once the patient Share ID is entered by the physician, it will be validated that the patient exists and a Response ID will be displayed. The Physician must confirm that the ResponseID is the one provided by the patient.

10. Once the assignment is successful, the new average daily dose will be available for the patient to retrieve using their MedsReducer-Patient App.

11. Corrections to the assignment may be made until the dose is retrieved by the patient. Once the dose is retrieved only average daily dose can be modified, if the starting date is in the future. The starting date cannot be changed once it has been retrieved by the patient.

12. The prescription button can be used from the main page to display a list of the 30 day requirements of each strength pill being used in the new average daily dose pattern. Note that the amount of the different strength pills used will vary with how many times a day the dose is to be administered. This feature aids the physician in determining what prescriptions to write.
